All payment is required at time of service. We are more than happy to provide you with an electronic super bill that includes both diagnostic and treatment codes you can submit directly to your insurance company for reimbursement.
Policy coverage varies depending on your plan. We suggest you contact your insurance company with the following questions:
How much will insurance cover once the deductible has been met?
How many visits are allowed per calendar year?
Are there restrictions to the coverage? (e.g. in lieu of anesthesia only, medical necessity)
Are there other limits or provisions with regard to acupuncture services?
Sometimes insurance companies request copies of your chart, treatment plan, and other notes as proof of “medical necessity” before sending you a check. We will respond to all requests in a timely fashion; however, please note that we charge a $25 administrative fee for submission of paperwork due to the amount of time necessary.
As part of your onboarding patient paperwork you will be asked to sign a "Release of Information" form to be kept on file. This allows us to send the documents required by the insurance company if requested.
